Stephen's sense of "this is all wrong" was pushed back pretty ferociously by "these feelings are Not Proper and my family will kill me." So he couldn't let the people around him know what was going on. But I figure at some point he moved into a new environment, realized he could present entirely as male from the get-go, and never looked back.
