How did Liz Lemon get to be queen of Comedy Central? How come Stephen didn't twig when she asked him to call her Liz? Or was that always meant to be someone different?

I truly loved this story, made me feel so bad for Stephen, and his possible child. His life with General O'Reilly was a horrible one.
